Role Description
This is a full-time role for a Nurse Care Manager located in the East Valley. The Nurse Care Manager will be responsible for providing comprehensive care management services to older adults and their families. Key responsibilities include assessing client needs, coordinating care plans, collaborating with medical providers, and ensuring the highest level of client care. The role also involves educating families about available care options and acting as their advocate throughout the process.
Qualifications
- Strong Nursing and computer skills to provide high-quality support and management
- Expertise in Nursing Management to develop, implement, and oversee care plans
- Background in home health, hospice or hospital nursing for effectively managing complex health conditions
- Empathy, excellent communication, and organizational skills to advocate for patients and support families
- Registered Nurse (RN) license or equivalent, with experience in geriatrics or a related field preferred
- Ability to work effectively on-site and collaboratively within a multidisciplinary team
- Working knowledge of aging care, mental health and developmental disability resources and services is an asset
