SUMMARY:

Under the direction of the Assistant Director of Vocational Programs, The Case Manager is responsible for providing comprehensive case management and employment services to individuals with disabilities. This role involves assessing clients' needs, developing individualized personal and employment plans, connecting clients with appropriate resources, and supporting them in achieving their personal and employment goals. The Case Manager will work closely with clients, families, funding agencies, and community partners to ensure success.

REQUIREMENTS:

· Bachelor’s Degree or commensurate experience (minimum four years) with developmentally disabled/mental health population or equivalent

· Valid California Driver's License/current insurance with a clean DMV record

· First Aid and CPR Certification with specialized certifications as required

· Proficient with data entry skills with Word, Outlook, and Excel programs

· Excellent verbal, writing, and organizational skills, Bi-lingual skills desired

· Attend all safety and required training classes

· Ability to multi-task and meet deadlines

DUTIES AND RESPONSIBILITIES: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

· Ensure client charts and records within the client software management system are current and compliant with all relevant regulations.

· Maintains confidentiality in accordance with HIPAA guidelines

· Perform thorough initial assessments to evaluate clients' abilities, skills, and both personal and employment goals.

· Demonstrate courteous and professional conduct and appearance while assisting clients and interacting with staff.

· Act as an advocate for clients within the program setting and in other relevant areas as needed.

· Work collaboratively with other departments, sharing information to support common clients.

· Ensure that clients receive accurate information, and present it in a way that enables them to make informed decisions and provide informed consent.

· Communicate professionally with the Program Manager, Director, funding agencies, and families as necessary.

· Collaborate within a team setting to establish and maintain appropriate goals and objectives for clients.

· Provide training to relevant staff on methods used to achieve client and program goals, ensuring that each client's program is effectively implemented.

· Participate in all client-related and departmental meetings as required.

· Regularly observe clients in their training programs to monitor progress and ensure program effectiveness.

· Oversee the intake process, ensuring it is conducted efficiently and effectively.

· Perform other duties as assigned

Work Environment:

This position requires community driving and supervising clients in a variety of public/community settings.

Physical Demands:

  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
  • The employee must have the ability to lift, pull, push, run and manipulate/use adaptive equipment and patients, which requires strength and gross motor and fine motor coordination. The employee must be able to perform CPR and administer First-Aid. The position requires sitting and/or standing for long periods of time. The position requires ability to operate an automobile, kitchen appliances, landscape equipment, and general maintenance equipment. The employee must be able to climb stairs and provide services to clients in various public/community settings.

Note duties are subject to change due to agency growth or restructure. Daily operations are also subject to change based on outcome measurements of systems incorporated to program operations

Pay: From $21.00 per hour
