This work includes: disease prevention, surveillance and control; food and consumer safety; assuring clean and safe drinking water; ensuring access to essential health services; providing education about healthy lifestyles; implementing strategies to reduce and prevent high-risk behaviors among youth; and responding to pandemics and emerging threats, outbreaks and other events.

ABOUT THE JOB:
We believe all families can thrive and deserve equitable access to healthcare and support services in their community. We value therapeutic relationships, evidence-based/informed care, and community partnership. The incumbent will work with external partners in the community, and pregnant women in a clinical setting providing PCC program services, referral coordination, and health promotion and education activities appropriate for the perinatal population. This position will be an integral part of the Family Support Services team, working in conjunction with home visiting nurses and perinatal care coordinators to improve the health outcomes of the individuals and families in our community.
This is a limited-duration, full-time Monday through Friday, 8am-5pm position which will be based onsite at the Bend Health Services building, and will also include work in the community and at other Health Services locations throughout the county. This is not a remote work position.
Key Responsibilities:
- Conducting clinic and telehealth visits with clients, connecting them with resources, and providing OHP enrollment assistance.
- Providing service navigation support, care coordination, and referrals for individuals and families to community resources.
- Documenting visit encounters to include assessment, planning, implementation, and evaluation of care in our EHR and data collecting systems.
- Establishing good working relationships with community partners, and promoting and informing them of perinatal services at Deschutes County Health Services.
- Managing external and internal referrals for perinatal services at Deschutes County Health Services.
Knowledge of or experience with:
- Passion for helping medically and socio-economically at-risk populations.
- Ability to work well with diverse populations through inclusive and culturally responsive practices.
- Understanding of the challenges some individuals and families face within our community and our health care system.
- Establishing and maintaining effective working relationships with other employees, clients and their families, other agencies, and the community.

Compensation
$5,657.33 to $7,581.35 per month for a 172.67 hour work month. Excellent County benefit package when eligible. This union-represented position is available immediately.

This is a limited-duration employment position as defined by Deschutes County HR Personnel Rules 2.010. The position has identified funding through October 31, 2027. Extending the position beyond October 31, 2027 will depend on a continued business need for the position and approval of additional funding. Should this occur, the County may at a later date extend the position end date or offer regular employment status to the incumbent in this position.
Minimum Qualifications
- Associate’s degree in a public health related field;
- AND one (1) year of related experience;
- OR any equivalent combination of training, education, and experience that provides the required skills and knowledge to perform the essential functions of the job.
- Bilingual in English and Spanish. (May be eligible for additional compensation.)
- Experience with OHP enrollment assistance.
- Knowledge of and experience using EPIC electronic health records.
Necessary Special Requirements
Driving is a requirement for this position. Possession of or ability to obtain a valid Oregon driver's license within 30 days of hire date. The employment offer will be contingent upon presentation of an acceptable and verifiable driver's license, pre-employment screening for criminal history, driving history, and controlled substances (NOTE: Positive test results for marijuana use may result in rescission of a contingent offer of employment). This screening must be completed with satisfactory findings in order for a formal offer of employment to be extended.
Must have dependable transportation and be able and willing to drive throughout Deschutes County in sometimes adverse weather conditions.
Must have training and certification as a Community Health Worker.Computer literacy with strong typing and computer skills is required.
