activities.
Job Description:
- Provide direct patient care in a home setting, ensuring the comfort and well-being of
patients
- Administer medications and treatments as prescribed by the physician (Wound Vac, IV
Certified, Wound care, etc.)
- Monitor and record vital signs, such as blood pressure, temperature, and heart rate
- Implements plan of care initiated by the registered nurse / Referring doctor
- Educate patients and their families on proper healthcare procedures and disease
management
- Provides accurate and timely documentation consistent with the plan of care
- Participates in coordination of home health services, appropriately reporting the
identified needs for other disciplines (HHA, OT, PT, MSW, ST,) to the registered nurse
and/or Clinical Supervisor/Nursing Supervisor
- Participates in personal and professional growth and development.
-Uses equipment and supplies effectively and efficiently.

Requirements:
- Valid nursing license in the state of California
- Minimum 1 year of experience as an LVN in Home Health
- Proficient in taking and interpreting vital signs
- Full time / Part time position
Salary : $35 - $45
