Dermedix Care is seeking a compassionate and motivated Nurse Practitioner (NP) to join our growing team serving patients throughout Milwaukee, WI and the surrounding areas.
This is a full-time position focused primarily on providing advanced wound care services in skilled nursing facilities, assisted living communities, and other long-term care settings.
The provider will manage a high volume of patients while delivering evidence-based wound care and helping improve patient outcomes. In addition to wound care, the provider will also participate in our Chronic Care Management (CCM) program by supporting ongoing care coordination for eligible patients.

Nurse Practitioner Duties and Responsibilities
The primary responsibility of the Nurse Practitioner is to provide the highest quality patient care while ensuring the best possible clinical outcomes through evidence-based treatment plans.
Responsibilities Included:
- Perform comprehensive patient evaluations and wound assessments.
- Develop individualized treatment plans based on clinical findings.
- Perform advanced wound care procedures, including debridement when appropriate.
- Assess patient vital signs and overall health status.
- Obtain and review patient medical history and current symptoms.
- Monitor and document wound healing progress.
- Maintain accurate electronic health records (EHR) and ensure documentation meets industry standards.
- Educate patients, families, caregivers, and facility staff regarding wound care, treatment plans, and preventative measures.
- Provide clear orders and prescriptions to pharmacies and staff
- Coordinate care with physicians, specialists, home health agencies, nursing staff, and other healthcare providers.
- Participate in Chronic Care Management (CCM) by reviewing patient care plans, coordinating ongoing care, and supporting chronic disease management for eligible patients.
- Communicate with patients / Caregivers/ facility staff, regarding appointments, follow-up care, and treatment recommendations.
- Travel between assigned facilities throughout the Milwaukee area.
Qualifications & Qualities
- Active Wisconsin Nurse Practitioner (NP) license.
- Valid driver's license and willingness to travel throughout the Milwaukee area.
- Ability to work independently while following established clinical protocols.
- Excellent communication and interpersonal skills.
- Compassionate, patient-focused, and service-oriented with a caring and professional demeanor.
- Patient, empathetic, and passionate about caring for the senior population.
- Strong patient education and counseling skills with the ability to effectively communicate treatment plans to patients, families, caregivers, and facility staff.
- Experience working with geriatric patients is preferred.
- Experience working in skilled nursing facilities, assisted living communities, long-term care facilities, or in pts homes
- Ability to build strong relationships with patients, families, facility staff, physicians, and other healthcare providers.
- Strong organizational, time management, and documentation skills.
- Experience using Electronic Health Records (EHR).
- Comfortable managing a high-volume patient schedule while maintaining exceptional quality of care.
- Experience in wound care is preferred but not required. Training will be provided to the right candidate.
- Experience with Chronic Care Management (CCM) is a plus but not required.
