Your Role as a Hospice RN Case Manager
As an RN Case Manager, you will coordinate and oversee hospice care for an assigned group of patients while providing skilled nursing, education, and emotional support to patients and their families.
Responsibilities include:
Provide compassionate hospice nursing care in patients' homes and other care settings
Complete comprehensive nursing assessments
Develop and update individualized plans of care
Manage symptoms with a focus on comfort and quality of life
Monitor patients for changes in condition and respond appropriately
Coordinate medications, treatments, supplies, and other patient needs
Educate patients and families about medications, symptom management, disease progression, and end-of-life care
Communicate with physicians and other members of the hospice team
Collaborate with CNAs, social workers, chaplains, and other interdisciplinary team members
Participate in interdisciplinary group (IDG) meetings
Provide emotional support and guidance to patients and their loved ones
Maintain timely and accurate clinical documentation
Help families understand what to expect and ensure they feel supported throughout the hospice journey
What We're Looking For
Current Arizona RN license or applicable compact license
Compassionate and patient-centered approach to care
Strong nursing assessment and critical-thinking skills
Excellent communication and organizational abilities
Ability to work independently in the field while collaborating closely with the hospice team
Reliable transportation
Hospice or end-of-life care experience preferred
