What are the responsibilities and job description for the Home Based Primary Care Nurse Practitioner position at Department of Veterans Affairs?
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Salary : $92,132.00 to $143,455.00 Per Year
Work Schedule: Monday-Friday, 8:00 am - 4:30 pm
Job Duties:
- The HBPC Nurse Practitioner assumes responsibility for the developing, interpreting and administering medical services to Veterans participating in the HBPC Program
- The HBPC Nurse Practitioner assesses the health status of Veterans by interviews, health histories, home visits, physical examinations and diagnostic tests.
- The incumbent assesses the physical and psychosocial health/illness status of individuals and groups, and promotes health maintenance, wellness and prevents illness.
- Plans are implemented through independent action, health counseling and collaboration with other members of the HBPC team.
- The primary focus of practice is to maintain optimum health, prevent illness and injury and manage selected acute or chronic health problems.
- The incumbent is responsible for providing assessment, home visits and medical care to Veterans participating in the HBPC program within the VA New Jersey HBPC Catchment area.
- The HBPC Program is within Extended Care service line. The HBPC Program consists of the HBPC, Medical Foster Home (MFH) and the Rural Health Programs.
- The Nurse Practitioner will also provide health-and-wellness psychoeducation and suicide/depression screenings to HBPC participants.
- Orientation will be at the Lyons Campus of the VANJHCS, but the HBPC Nurse practitioner must possess a valid driver's license in order to conduct Home care assessments and provide care at other locations in the community.
Preferred Experience: Active Drivers License as the job requires travel; Nurse Practitioner 5 years experience
