General Description

The Collaborative Care Case Manager (CCCM) works under the leadership of the Program Manager of IDD Crisis and oversees intensive case management provided to individuals served in the Outpatient Biopsychosocial Interventions program. The CCCM ensures a collaborative approach to providing individuals with IDD and co-occurring mental health and/or substance use conditions with case management, skills training, specialty providers and other providers to ensure accurate diagnosis and supportive treatment planning. This position provides the biopsychosocial assessment, development of the person centered recovery plan, facilitates service linkages, and provision of crisis intervention/prevention supports. This position requires the ability to communicate effectively, complete required documentation and the ability to maintain a positive relationship with clients, families and service providers.

Individuals served through OBI will have IDD and co-occurring mental health needs such as severe mental illnesses, substance use disorders and/or chronic health conditions. OBI staff operate in a dynamic environment, develop extensive knowledge of community resources, and collaborate with interdisciplinary teams. Interventions and services are typically provided in the community or wherever necessary to facilitate progress for the individuals served.

Education, Training, and Experience

  • A bachelor's or advanced degree from an accredited college or university with a major in a social, behavioral, or human service field including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice is required.
  • At least one year of experience with individuals with intellectual or developmental disabilities is required. Additional experience with individuals with mental illness is preferred.

Registration, Certification, Licensure, and Other Qualifications

  • Must have and maintain a background and criminal history free from any disqualifying offenses as outlined by the Texas Administrative Code (TAC) and the Health and Human Services Commission (HHSC).
  • Must possess and maintain a valid driver's license and automobile insurance.
    • Individuals with an out-of-state driver's license must be able to obtain a driver's license in the state of Texas within thirty (30) days.
  • Successful completion of all position-specific training within thirty (30) days of employment and ongoing compliance with annual training requirements is required.

Knowledge, Skills, and Abilities

  • Knowledge of intellectual and developmental disabilities and services available in and near Denton County.
  • Ability to interact with potentially aggressive individuals.
  • Ability to work with individuals who are incarcerated or involved in the justice system.
  • Ability to develop, implement, and evaluate therapeutic support plans for each Individual.
  • Skills include assessment of individual support needs, historical information, and identifying treatment goals.
  • English/Spanish bi-lingual preferred.
  • Ability to work independently and make informed decisions quickly.
  • Ability to display regular and reliable attendance.
  • Ability to gather, analyze, and report data.
  • Ability to communicate professionally in written and oral communication.
  • All job specific training must be completed within 30 days of hire
  • Understanding and practice of Trauma Informed Care principles.

Essential Duties

The following list outlines key responsibilities for the position; however, it is not exhaustive and does not encompass all responsibilities. Additional duties may be assigned as needed.

1. Perform assessments necessary for enrollment into the program in a timely manner.

2. Establish and maintain the person centered recovery plan for individuals served.

3. Perform intensive case management for individuals assigned to the caseloads.

  • Enrolled participants will need to be seen at least twice a month while receiving services through the OBI program.

4. Complete skills training activities with enrolled individuals in line with their treatment goals.

  • Skills training must be completed at least one a month and must align with the individual's needs and goals.

5. Collaborate with appropriate LIDDA and LMHA staff, Transition Support Team members, Community Resource Committee Group members, and any other community members or resources that could be beneficial for the individuals served.

6. Attend weekly team meetings.

7. Complete documentation in the Electronic Health Record within two business days of any meetings.

8. Adhere to Texas Administrative Code guidelines and requirements.

9. Participates in the Center's Zero Suicide Initiative.

  • Supports individuals experiencing a crisis by promptly connecting them with appropriate Center programs, such as the Mobile Crisis Outreach Team (MCOT), Psychiatric Triage, Crisis Prevention services, and/or other applicable resources. Ensure seamless coordination to facilitate timely intervention.
  • Remains with individuals, when deemed safe, to provide support and maintain a calm environment until specialized crisis staff or emergency personnel arrive to take over intervention efforts. Adheres to safety protocols to prevent escalation of the crisis situation.
  • Participates in debriefing sessions with the immediate supervisor or designated staff following a crisis event. Collaborate with the team to review the incident, identify lessons learned, and ensure all HHSC guidelines and Center policies are followed.
