Nurse Practitioner (Health Programs)

Reporting to:   Director of Primary Care and Medical Director

SUMMARY:  The Primary Care Nurse Practitioner (NP) provides comprehensive, patient-centered primary care services to individuals and families across the lifespan in a Federally Qualified Health Center (FQHC) setting. The FNP delivers preventive, acute, and chronic care using evidence-based practices while supporting the organization’s mission to improve access to high-quality, culturally competent healthcare for underserved and diverse populations. This role functions as an integral member of a multidisciplinary care team, collaborating with clinical, behavioral health, and support staff to promote continuity of care, population health outcomes, and compliance with HRSA and other regulatory requirements.

The Primary Care NP demonstrates strong clinical judgment and diagnostic skills, with the ability to assess patients, interpret diagnostic results, develop treatment plans, prescribe medications, and provide health education and counseling. The position requires effective communication and interpersonal skills, a commitment to patient-centered and trauma-informed care, and the ability to work efficiently in a fast-paced ambulatory care environment. The Primary Care NP must be adaptable, organized, and capable of balancing quality care, productivity expectations, and timely, accurate electronic health record documentation. Knowledge of primary care, public health principles, and culturally responsive care for medically underserved populations is essential.

The work environment is a clinical outpatient setting. The role requires the ability to sit, stand, walk, and move throughout the clinic for extended periods, as well as the ability to perform physical examinations and procedures. Occasional lifting of up to 25 pounds may be required. The position may involve prolonged screen time for documentation and patient communication and regular interaction with patients, staff, and community partners.

Minimum Qualifications

  • Master’s or Doctoral degree in Nursing from an accredited program.
  • Current certification as Nurse Practitioner (ANCC or AANP) in Family (FNP), Pediatric (PNP), or Women’s Health (WHNP).
  • Active, unrestricted state APRN license.
  • DEA registration (or eligibility to obtain).

 

 Essential Duties:

 

 

  • Provide comprehensive primary care services to patients of all ages, including preventive care, diagnosis, treatment, and management of acute and chronic conditions.
  • Perform patient assessments, physical examinations, and medical histories.
  • Order, interpret, and evaluate diagnostic tests (labs, imaging, etc.).
  • Prescribe medications and treatments in accordance with state regulations and organizational protocols.
  • Provide health education, counseling, and disease prevention services.
  • Deliver care in accordance with evidence-based guidelines and quality standards.
  • Serve patients regardless of insurance status or ability to pay, in alignment with FQHC mission and values.
  • Provide care consistent with HRSA requirements, Uniform Data System (UDS) measures, and FQHC clinical guidelines.
  • Support care for vulnerable populations, including uninsured, underinsured, migrant, homeless, and medically underserved individuals.                                                                                                                                   
  • Participate in enabling services such as care coordination, patient navigation, and referrals.
  • Collaborate with physicians, behavioral health providers, dentists, nurses, medical assistants, case managers, and other care team members.
  • Participate in interdisciplinary team meetings and quality improvement initiatives.
  • Coordinate referrals to specialty care and follow up on patient outcomes.
  • Maintain timely, accurate, and complete documentation in the electronic health record (EHR).
  • Ensure compliance with federal, state, local, and organizational policies, including HIPAA and OSHA requirements.
  • Support clinical quality initiatives, risk management activities, and performance improvement efforts.
  • Maintain current licensure, certification, and continuing education requirements.
  • Participate in required trainings, staff meetings, and ongoing professional development activities.
  • Leadership by example and ability to communicate appropriate needs with administration and staff.
  • Outreach to the community, area providers, hospitals, and clinics as needed for patient referrals in accordance with FQHC standards.
  • Provide care on mobile unit, health fairs, schools and other businesses as needed.
  • Other duties as assigned by management.
