Community Health Worker – New You!
Reports to Director of Programs
MINIMUM REQUIRMENTS:
- Minimum of AA degree.
- Two (2) years’ experience in a related field
- Two (2) years or more as resident in Washington County and working knowledge of community resources and events.
- Proficient computers skills, including Microsoft, specifically Word and Excel, in addition to excellent oral and written communication skills.
- Must have a valid Mississippi Driver’s License, proof of insurance and reliable transportation
RESPONSIBILITIES:
- Conduct individual home visits with the participants working on losing weight and changing mindset.
- Manage caseload (up to 75), keeping accurate records and ensuring that all related administrative tasks are completed and up to date.
- Provide the evidence-based curriculum and emotional support.
- Maintain an effective support service for participants with the aim of enabling participants to lose weight and helping with any resources necessary to help their develop of a healthier lifestyle.
- Design and participate in recruitment activities especially promoting increased awareness of services by educating the public and linking with other programs.
- Maintain confidentiality of all participants; communicate orally and in writing to parents, professionals,
- and other agencies.
- Plan and implement activities and educational classes for participant that are culturally sensitive and developmentally appropriate both in individual and group settings.
- Other duties as assigned.
