What are the responsibilities and job description for the Medical Assistant II - Delta Family Medicine position at Delta County Memorial Hospital?
- Coordinate patient care plans by liaising with all necessary care providers and medical professions to ensure patient needs are met. Developing health care programs and help expedite client services referrals.
- Conduct intake assessment, needs assessment, treatment planning and reassessment services.
- Screen patients for eligibility for direct and support services and refer patients to needed services.
- Maintain documentation of all patient encounters and complete reporting requirements according to organization standards.
- Track patient information, schedules, files, and forms in a confidential manner.
- Meet with patients and families regarding treatments, procedures, medications, and continuing of care.
- Discuss cost of services, insurance coverage, and payment options with the patient. Responsible for a minimum of six (6) patient annual wellness visits weekly.
- Communicating between staff, patients and family.
- Handling patient case management and education.
- Developing patient goals and monitoring progress.
- Interviews patients, measures vital signs, such as pulse rate, temperature, blood pressure, weight and height, and records information on patient’s charts.
- Makes phone calls to patients regarding test results or medication changes.
- Administers injections and medications, dresses wound and incisions.
- Rooms patients, obtains clinical information such as medication list, patient concerns, questions and height and weight.
- Maintains skill level required to care for routine patients.
- Anticipates patient needs and responds to patient requests promptly.
- Able to successfully perform blood draws.
- Able to enter information in EMR.
- HIPPA compliance, facility compliance.
- Help patients’ complete paperwork, communicating with a healthcare team about a patient’s treatment plan and educating patients about resources and options for managing their health.
- High School graduate or equivalent required.
- Current Certification as a Medical Assistant required.
- Superior interpersonal communication skills and ability to exercise empathy when working with patients and their families
- Strong organizational and time management skills.
- Demonstrate an understanding of standard clinical procedures, laws and regulations.
- Thorough knowledge of medical terminology.
- Ability to speak a second language is an asset.
- BLS Certification Required.
Medical Assistant II is responsible for identifying and facilitating treatment plans for patients with chronic care conditions. Review recommendations from the patient’s health care providers and coordinate the treatment and community resources available to the patient. In addition, the MA II is responsible for performing a target number of Medicare annual wellness encounters weekly. Must comply with HIPAA and other federal, state and local regulations, as well as maintain highest degree of confidentiality in client and staff matters.
