COMPENSATION AND BENEFITS Primary Care Physician (FNP, AGNP)
- Up to $140K annual salary
- $20K Signing bonus
- Up to $25K in Annual year-end bonuses

RESPONSIBILITIES AND FACILITY DETAILS Primary Care Physician (FNP, AGNP)- 100% Outpatient - primary focus on Geriatrics patient-population
- Preventative care focus on Chronic conditions to reduce hospitalization
- Well trusted, Physician-lead healthcare org. with over 120 locations in 15 states
- Patient-focused, small patient panel – 450 patients max
- Monday – Friday shift-schedule, No Weekend
- Dedicated care team for full support, as well as On-site lab, Pharmacy, and X-Ray
QUALIFICATIONS AND SKILLS Primary Care Physician (FNP, AGNP)- BC / BE - FNP and AGNP providers welcome to apply
- 2 years of practice experience preferred
- Seeking bi-lingual providers for patient-needs - fluent in Spanish
