Position Summary
Responsible for the coordination of day programming services for adults with intellectual and developmental disabilities. Responsible for functioning as the Program Specialist for a caseload of individuals within the day program setting.

Principal Duties and Responsibilities
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Promote continuous quality improvement projects and ongoing performance measures.
Monitor the development and implementation of high quality individual support plans.
Coordinate the following with the Support Coordinator and planning team:
Annually provide an assessment for the development of the ISP
Annually update lifetime medical history
Develop outcomes
Complete an Initial Assessment in collaboration with the Program Supervisor within thirty (30) days of an individual starting the program and annually thereafter.
Monitoring the documentation completed by the Direct Support Professional of individuals participation and progress towards outcomes
Ensure supervision and all support needs as per ISP
Develop quarterly reviews
Providing training to staff in the following areas:
Day Programming procedures
Implementation of the ISP
Behavior Support Plans or Social Emotional Environmental Plans
Feeding protocols
Supervision levels
Conducting meaningful group activities
Communicate with families, providers and Support Coordinators regarding the individual’s progress and participation in the program.
Develop and provide meaningful activities that are person-centered in accordance with the individual plans.
Support the integration of individuals in the community.
Providing direct care as needed to ensure the health and safety of individuals we support.
Prepares for, and promptly responds to, emergency situations to ensure the safety and security of the Program site and individuals supported at all times; e.g., participates in fire drills, emergency evacuations, etc. Notifies proper authority in the event of a valid emergency.
Perform other duties assigned.
Qualifications:
Requirements
Ability to comprehend, read, write and speak English
Ability to use excellent oral and written communications skills.
Ability to use telephone, voice mail, copier, facsimile machine, computer and calculator.
Ability to work effectively with people both one-on-one and in a team environment, facilitating their growth and development.
Ability to resolve non-routine situations properly.
Ability to be observant and intuitive.
Ability to be dependable and punctual.
Ability to work on assigned tasks as well as to accept direction on given assignments.
Ability to operate vehicles with a valid driver’s license in accordance with applicable state laws, insurance provider requirements and company policies and procedures.
Position Qualifications
Master’s Degree or above from an accredited college or university and; one (1) year work experience working directly with persons with intellectual disabilities;
Or, a Bachelor’s Degree from an accredited college or university and; two (2) years’ work experience working directly with persons with intellectual disabilities;
Or an Associate’s Degree or sixty (60) credit hours from an accredited university or college and four (4) years’ experience in the supervision of community based programs working directly with persons with intellectual disabilities and/or autism.
Prior experience working with individuals who are medically/behaviorally acute.
Prior experience working with aging adults and adults with functional impairment.
