As the central point of contact for county agencies, you will manage the intake and placement process with precision, empathy, and urgency. You will ensure that every placement decision not only meets rigorous state and county regulatory standards but also prioritizes the unique emotional and developmental needs of the youth we serve.
Key Responsibilities

Strategic Matching: Facilitate the placement of children and youth referred by County Children and Youth Agencies into appropriate, certified Delta resource homes.

Compliance Oversight: Maintain strict adherence to the PA Department of Human Services regulations and specific County service performance standards.

Case Coordination: Review referral packets to assess the needs of the youth and identify the resource parents best equipped to provide a supportive environment.

Relationship Management: Act as the primary liaison between county caseworkers and Delta’s clinical and certification teams to ensure a seamless transition for the child.

Quality Assurance: Uphold agency guidelines to ensure all placements promote safety, permanency, and well-being.

Experience & Knowledge
