What are the responsibilities and job description for the Clinical Liaison Rehab Admissions Per Diem Days position at Delray Medical Center?
Position Summary:
The Clinical Liaison functions as the liaison between patient, family (and when indicated legal representatives), referring hospital, and Delray Medical Center-Pinecrest Rehabilitation unit prior to admission. The Clinical Liaison is responsible for facilitating the delivery of the most appropriate post-acute rehabilitative services by accurately collecting data for pre-admission review, effective management of available resources, identifying an interdisciplinary treatment plan with realistic discharge goals of length of stay and outcome. Acts as a liaison between the referral source(s), physicians, Rehab Director, admitting office, nursing, inpatient case management and Manager of Psychology. Is responsible for obtaining the required documentation needed related to appropriate CMS documentation to meet admission criteria and the CMS-13 rule. The Clinical Liaison educates the professional community on topics related to rehabilitation and participates in coordinating and participating in community lecture series and health fairs.
Position Qualifications:
EDUCATION: Graduate from an approved/accredited program.
Minimum: RN, PT, OT, ST, or other related health field.
EXPERIENCE: Minimum: One (1) year previous clinical experience providing direct case management or pre-admission assessments for rehab or home health or two (2) years general experience in health care field.
REQUIRED CERTIFICATIONS/ LICENSURES/ REGISTRATIONS:
RN (CRRN preferred), PT, ST, or OT; Must have current Florida Professional license.
