Behavioral Health Case Manager
The Care Support Case Manager provides an integral part of the support system available to our clients who participate in the specialized treatment programs (PHP and IOP) at Delray Center for Recovery. They assist clients with the transition to the program, striving to make the process as smooth and stress free as possible. Someone with Mental Health experience is preferred. Must have interest in Mental and Behavioral Health.
Status: Hourly, Non-Exempt, Full-Time
Essential Job Functions:
Client Care
- Guide new clients through the program orientation process
- Work with client’s therapy team to create and arrange the appropriate weekly schedule
- Meet weekly with each client for case management to discuss any concerns and answer questions regarding program procedures and resources
- Serve as a liaison between the client and his/her therapy team in between therapy sessions as well as to serve as a Behavior Health Technician and report on any behavioral concerns
- Monitor client participation and therapeutic homework in the milieu
- Distribute, compile, and complete/document informal assessments and inventories
- Coordination of drug and alcohol screenings
- Plan and organize community client events, encourage client participation, and disperse this information to the rest of the team
- Manage/Supervise client program meals and event catering
- Assist with the planning and scheduling of group therapy and workshops
- Assist clients with referrals to outside services and community resources when necessary and as advised by the client’s primary therapist for case management services
- Assist with any Grievance complaints or procedures as deemed necessary
- Work alongside clinical team to confirm renewal process
- Responsible for program phone communication
- Maintain updated knowledge of all programs and services
- Assist group therapy clients with registration and group information
- Communicate scheduling changes to clients and care-takers
Administration
- Prepare updated group therapy schedules and distribute to appropriate departments
- Prepare daily and weekly program client status reports
- Open and close building on schedule
- Maintain a welcoming, clean, and healing environment
- Attend staff meetings and training programs as required by management
- Formulate daily client documentation and work with primary therapist to complete relevant surveys and documentation for program admissions, step downs, and discharges
Team Support
- Assist and support other team members in their efforts to provide exceptional service and care to each individual client including providing supervision for clients with high needs
- Assist Business Manager with client admissions and program related needs
- Assist Clinical Director and therapists as needed (This may include periodic group coverage)
Preferred Education & Qualifications
- Authorized to work in the United States
- Must be age 18 or older
- 2 years experience in mental health setting or interest in pursuing a career in mental health field preferred
- Ability to case manage clients with high mental health needs
- 2 years experience with customer service or in a medical office
- 2 years of related college coursework, preferably a Bachelors in related field of study
- Proficient in Microsoft Word & Excel or similar software
- Good computer skills, preferably with Mac
- Working knowledge of Electronic Medical Records software
- Excellent communication and time management skills
- Ability to organize and manage multiple tasks effectively
- Must pass drug test
- Must pass criminal background check
Job Type: Full-time
Pay: $19.00 - $22.00 per hour
