MDS Coordinator Major Tasks and Responsibilities:
Coordinate and complete MDS assessments in accordance with CMS guidelines and facility policies.
Ensure timely and accurate completion of all MDS, Care Area Assessments (CAAs), and care plans.
Collaborate with interdisciplinary team members to develop and update individualized resident care plans.
Monitor clinical documentation to support MDS coding accuracy and reimbursement.
Review resident records and conduct assessments to determine care needs and services.
Track assessment schedules and maintain compliance with required submission deadlines.
Participate in Medicare, Medicaid, and insurance reviews as needed.
Educate nursing staff regarding documentation requirements and regulatory compliance.
Monitor quality measures and identify opportunities for clinical improvement.
Assist with survey preparedness and regulatory audits.
Collaborate with facility leadership to ensure optimal reimbursement and resident outcomes.
Maintain confidentiality of resident information in accordance with HIPAA regulations.
MDS Coordinator Required Education / Experience:
Current Registered Nurse (RN) license in good standing.
Minimum of 1-2 years of MDS experience in a skilled nursing or long-term care setting.
Strong knowledge of MDS 3.0, RAI process, and Medicare/Medicaid regulations.
Excellent assessment, organizational, and communication skills.
Ability to work independently and manage multiple deadlines.
Salary : $100,000 - $105,000
