Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by taking vital signs such as blood pressure, pulse, and temperature; report changes to healthcare professionals
- Support medication administration following care plans and HIPAA regulations to ensure privacy and safety
- Provide companionship and emotional support to reduce feelings of isolation and promote mental well-being
- Serve nutritious meals tailored to individual dietary needs.
- Maintain a clean and safe environment through regular cleaning duties and proper infection control practices
- Observe and document patient conditions meticulously, noting any changes in behavior or health for ongoing care planning
- Support clients in community outings or social activities to foster engagement and independence
- Follow all safety protocols including CPR, First Aid, and dementia or Alzheimer’s care procedures to ensure client safety at all times
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care settings is highly preferred
- Demonstrated knowledge of patient observation techniques, medication administration, and vital signs monitoring
- Familiarity with HIPAA compliance, patient monitoring tools, and care plan implementation is essential
- Ability to perform physical tasks including heavy lifting and the use of assistive devices such as Hoyer lifts or wheelchairs
- Previous roles involving hospice care or long-term care environments will be considered a plus
- Certification in CPR and First Aid will be required; additional certifications in nursing assistance or social work are beneficial
