What are the responsibilities and job description for the Hospital Liaison position at DELAWARE HOSPICE INC?
Delaware Hospice is actively seeking a full-time Hospital Liaison which will cover locations in Kent and Sussex Counties.
The Hospital Liaison functions as a clinical and business development partner within assigned healthcare systems, service areas, and growth markets, serving as a trusted representative of Delaware Hospice. The position combines clinical expertise, timely patient assessment and admission support with relationship development, care-transition collaboration, and identification of opportunities to strengthen access to Delaware Hospice’s full suite of services, including palliative care, dementia care, Katybug pediatric support, and hospice care. The Hospital Liaison works collaboratively with hospital leaders, physicians, case management, social work, nursing, palliative care, and other key stakeholders to improve access to the most appropriate level of care, facilitate appropriate transitions, and strengthen long-term partnerships between Delaware Hospice and healthcare systems across assigned geographies.
Delaware Hospice Suite of Services: The Hospital Liaison demonstrates working knowledge of and promotes appropriate access to Delaware Hospice’s services across assigned areas, including palliative care for individuals with serious illness, dementia care support for patients and families navigating cognitive decline, Katybug pediatric support for children and families, and hospice care for patients who meet eligibility criteria and need end-of-life support.

Qualifications:- Current professional nursing license in the state or jurisdiction required for the assigned service area, with ability to meet licensure requirements for additional markets as expansion needs evolve.
- Graduate of an approved school of nursing.
- Bachelor's degree in nursing preferred.
- Certification in hospice nursing preferred.
- Minimum of two years of nursing experience, preferably in hospice, home care, hospital, or another healthcare setting.
- Strong communication, relationship-building, collaboration, organization, and presentation skills.
- Ability to work effectively with physicians, healthcare-system leaders, interdisciplinary teams, patients, and families.
- Ability to understand healthcare-system priorities and translate clinical expertise into collaborative, patient-centered solutions.
