What are the responsibilities and job description for the Support Coordinator/Intake Coordinator position at DeLA Case Management?
The Support Coordinator will conduct initial CPOC meetings, Discovery meetings, and complete the LaPLUS assessment and develop the initial CPOC. The ideal candidate will be able to take participants from linkage to a finalized CPOC. Prior experience completing the initial plan of care (with all required elements) required. The applicant will complete initial plans across all OIDD waivers (Children's Choice, Supports Waiver, ROW Waiver and NOW). The applicant should be familiar with the OIDD "bump" request procedure as well. DeLA covers the entire Region 6 area (Avoyelles, Catahoula, Concordia, Grant, LaSalle, Rapides, Vernon and Winn). Reimbursed travel required throughout Region VI. At least a Bachelor's degree in Human Services Field required. Previous experience as Support Coordinator required. Support Coordinators are eligible to work from home after 60 days of employment. Qualified Support Coordinators are eligible for pay increase after first 90 days. Applicants receive sign-on bonus after 3 months of successful employment. Computer skills (Microsoft Word, Exel, Powerpoint) required. Compensation based on experience.
