Investigate reports of abuse, neglect, and exploitation involving adults over 60 years of age. Assessing safety and well-being. Develop and implement protective services to ensure the adults immediate and long-term safety.
Maintains a caseload of children and families who have previously been determined to require protective ongoing services of the agency as assigned. Provides case management services to protective and voluntary cases, including completing home visits as determined appropriate. Attends all court hearings, informs, and recommends to client services and agencies needed for successful outcomes. Provides or facilitates independent living services for children in custody. (Protective and voluntary included)
Prepares case plans and all required review documents, attends review meeting and hearings and conducts home visits with foster parents, children, and biological parents/guardians. Completes all necessary documents and data entry into SACWIS.
Serves as on-call social services worker (weekends, holidays, after hours, investigation claims of abuse and neglect), as assigned, or scheduled and adheres to on-call guidelines at all times.
Investigates claims of abuse and neglect of children. Responds to location of report within timeliness established in ORC/OAC. Ensures safety of alleged victim(s), works with law enforcement, courts, and the prosecutor’s office as needed. Completes all necessary documents, safety plans, risk assessments, etc. and data entry into SACWIS. Works complete investigation in the mandated time frame.
Provides supervisor consistent updates on case management activities. Meets procedural guidelines governing the management of social service cases and ensure that documentation is complete. Participates as requested in State reviews and provides all necessary information and documentation.
Rotates with other staff, the in-office intake duties including required documentation etc.
May be asked to make speeches and talk to civic and/or school groups to educate them as to social services programs.
Determines eligibility for PRC, provides/facilitates PRC social services.
Other duties as assigned by supervisor and/or directors.
