MDS Coordinator (RN or LPN)

Deerfield Nursing & Rehabilitation Center is seeking an experienced, detail-oriented MDS Coordinator to join our leadership team. The ideal candidate will have a thorough understanding of the MDS process, Medicare and Medicaid regulations, reimbursement, and interdisciplinary care planning.
Responsibilities:
- Coordinate and complete accurate and timely MDS assessments in accordance with federal and state regulations.
- Ensure compliance with CMS guidelines and RAI Manual requirements.
- Manage Medicare and PPS assessments while maximizing appropriate reimbursement.
- Collaborate with nursing, therapy, dietary, social services, and other departments to develop comprehensive care plans.
- Monitor assessment schedules and ensure all required documentation is completed accurately.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities.
- Assist with survey readiness and regulatory compliance.
- Educate and support interdisciplinary team members regarding MDS documentation requirements.
- Review medical records for accuracy and completeness.
Qualifications:
- Current Louisiana RN or LPN license in good standing.
- Previous MDS Coordinator experience preferred.
- Strong knowledge of CMS regulations, Medicare, Medicaid, and PDPM.
- Excellent organizational, communication, and critical thinking skills.
- Ability to work independently while collaborating with the interdisciplinary team.
- Proficient in electronic medical record systems.
