      • Work may involve occasional travel between physician practice locations, hospital departments, community sites, and attendance at meetings or training sessions.

      • Full-time days, Monday through Friday

        Generally works scheduled physician practice operating hours. May be required to work occasional extended hours, evenings, or weekends based on clinic and organizational needs.

        Work schedule may be adjusted as needed to meet operational and business requirements.

    • Position Summary
      • The Registered Nurse, Physician Practice provides professional nursing care within an outpatient physician practice setting. The RN serves as a clinical resource for providers, patients, and staff by performing patient assessment, triage, care coordination, population health activities, patient education, and clinical follow-up. The RN utilizes clinical judgment to support quality patient outcomes, care continuity, regulatory compliance, and value-based care initiatives while collaborating with providers, medical assistants, care coordinators, and other members of the healthcare team.

  • KNOWLEDGE, SKILLS, & ABILITIES/POSITION QUALIFICATIONS
    • Education
      • Graduate of an accredited professional nursing education program required

        Associate Degree in Nursing (ADN), Bachelor of Science in Nursing (BSN), or equivalent nursing education qualifying the individual for RN licensure required

    • Experience
      • Previous ambulatory care, physician practice, primary care, care coordination, or population health experience preferred

    • Computer Skills
      • Proficient in Microsoft Office, email, and other standard computer applications, with the ability to learn and effectively use software systems required for assigned responsibilities.
      • Proficiency with electronic health records (EHRs)

    • Certificates & Licenses
      • Current Registered Nurse license in the State of Indiana

        Current BLS certification required

    • Other Requirements
        • Frequent use of computers, electronic medical records (EMR), telephones, and other office and clinical equipment.
        • Ability to work effectively in a fast-paced environment, prioritize multiple competing responsibilities, manage frequent interruptions, and respond promptly to patient, provider, and operational needs.
        • Ability to exercise sound clinical judgment, maintain attention to detail, and collaborate effectively as a member of an interdisciplinary healthcare team.
        • Ability to adapt to changing priorities, manage fluctuating workloads and high patient volumes, respond appropriately to urgent clinical situations, and maintain productivity and quality of care.
        • Use of appropriate personal protective equipment (PPE) and adherence to infection prevention and safety protocols.

        Essential Functions

        • Conducts comprehensive assessments of patients' medical, behavioral, functional, and support needs to identify barriers to achieving optimal health outcomes.
        • Develops, implements, and monitors individualized care plans in collaboration with patients, families, providers, and interdisciplinary team members.
        • Coordinates care across the continuum, including primary care, specialty care, hospital services, community resources, and ancillary providers.
        • Facilitates referrals, diagnostic testing treatment services, and follow-up appointments to ensure timely and appropriate patient care.
        • Monitors patient progress and treatment adherence, intervening as needed to address changes in condition, barriers to care, or unmet needs.
        • Supports chronic disease management initiatives by providing ongoing coaching, education, and care coordination for high-risk and complex patients.
        • Collaborates with providers and care teams to promote evidence-based care, improve patient outcomes, and reduce avoidable utilization.
        • Conducts patient outreach and follow-up through telephone, electronic communication, and other approved methods to support continuity of care.
        • Coordinates transitions of care, including post-discharge follow-up, medication reconciliation support, and linkage to community-based services.
        • Identifies patients due for preventive screenings, annual wellness visits, immunizations, and other recommended healthcare services to improve compliance with organizational quality measures and care standards.
        • Reviews and manages patient portal messages and clinical communications within the assigned provider inboxes, assesses patient concerns, and determines appropriate clinical action using professional nursing judgment.
        • Maintains accurate and timely documentation within the electronic medical record, including health maintenance records, care plans, interventions, and outcomes.
        • Reviews laboratory, diagnostic, and other clinical results that are routed to providers and ensures providers are reviewing in a timely manner.
        • Educates patients and families regarding diagnoses, treatment plans, medications, self-management strategies, preventive care, advance directives, and available community resources.
        • Reviews and processes incoming clinical records, consultation reports, specialty notes, hospital records, and other medical documentation, as well as ensuring problem lists, medical histories, and other clinical information are updated in the patient's medical record to remain accurate and current.
        • Other duties as assigned.
