What are the responsibilities and job description for the Evening Caregiver position at DDMS LLC?
Fern Park is looking for afternoon and evening Caregivers to assist our residents. You will provide basic care and active treatment to meet the needs of intellectually and developmentally disabled individuals in a home-like setting, in the Fern Park, FL area. We are looking for dedicated and reliable staff to fill several positions in our facility. We currently have multiple shifts open, 3:00pm-11:00pm.
Responsibilities:
• Assists and supports individuals in the implementation of their Individual Support Plans (ISP's) and activity schedules.
• Assists and supports individuals in performing daily living functions including bathing, meal time and toileting needs.
• Assist and supports individuals in all housekeeping duties to ensure a clean and pleasant living environment.
• Monitor individuals and the surroundings for appropriate safety and health care practices.
• Assist, coordinates and transports individuals to recreational activities in their homes and in the
community.
• Completes documentation regarding programming, health and safety, comprehensive assessments, and
other areas as assigned.
• Monitors individuals and their environment for any change in health, mental well-being, and behavioral well-being.
• Assist and supports individuals in implementing their Behavioral Support Plans as developed.
