What Youll Do
Provide direct patient care in alignment with the State Nurse Practice Act
Deliver skilled nursing care based on comprehensive assessments and individualized plans of care
Document patient services accurately and promptly
Educate patients and families on diagnoses, care plans, and symptom management
Collaborate with the interdisciplinary team to ensure coordinated, holistic care
Provide effective pain and symptom management and evaluate patient responses
What You Bring
Graduate of an accredited practical or vocational nursing program
Three years of nursing experience (hospice, community health, or med-surg preferred)
Current LPN/LVN license in the state of practice
Current CPR certification
Strong communication and organizational skills
Valid drivers license and insured, reliable vehicle
Understanding of the hospice philosophy and the needs of the terminally ill
