Nurse Practitioner - Family Practice

Full-Time | Permanent Position | Mid-Level Provider

We are currently seeking a dedicated Nurse Practitioner to join our Family Practice team. This is a full-time, permanent opportunity to float between two clinic locations in a community-based primary care setting.

Candidates: We are ready to provide training for all new Graduates!!!

• 1–3 months of structured training
• Shadowing experienced providers
• DEA license covered by the facility (not required at time of hire)

Responsibilities & Duties:

• Identifying short-term and long-range patient care issues that must be addressed; providing information and commentary pertinent to deliberations; recommending options and courses of action; and implementing provider directives.
• Assesses patient health by interviewing patients, performing physical examinations, and obtaining, updating, and studying medical histories.
• Determines abnormal conditions by administering or ordering diagnostic tests, such as x-rays, electrocardiograms, and laboratory studies; interpreting test results.
• Documents patient care services by charting in patient and department records.
• Instructs and counsels patients by describing therapeutic regimens; giving normal growth and development information; discussing family planning; providing counseling on emotional problems of daily living; promoting wellness and health maintenance.
• Provides continuity of care by developing and implementing patient management plans.
• Maintains a safe and clean working environment by complying with procedures, rules, and regulations.
• Protects patients and employees by adhering to infection-control policies and protocols.
• Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; and advising management on needed actions.
• Nurse Practitioner maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.

Required Qualifications:

• Active Illinois Nurse Practitioner license
• Master’s or Doctorate from an accredited Nurse Practitioner program

Compensation

• $110,000 – $125,000 (first year)
• Competitive salary based on experience
• $250 monthly stipend toward private health insurance
• No hospital or off-site rounds required

Schedule

• Monday: 10:00 AM – 7:00 PM
• Tuesday–Thursday: 10:00 AM – 6:00 PM
• Friday: 9:00 AM – 6:00 PM
• Saturday: 9:00 AM – 3:00 PM
• Sunday: Closed
• 3 out of 4 Saturdays per month (weekday off provided when working Saturday)

Patient Population

• Walk-ins from the community
• All age groups
• Family Practice / Primary Care focus
• Spanish-speaking patients (Bilingual Spanish is a major plus)
