Driver files:
- For new drivers:
- Make sure driver completes all necessary paperwork
- Follow FMCSA guidelines to contact previous employer regarding driving record and testing results
- Copy of driver record from state agency
- Send driver for initial drug screen
- Review DMP rules, code of conduct and drug policy – have them sign acknowledgement
- Obtain copy of medical card and driver’s license
- Prior to first drive, send for alcohol screen
- DMP driver agreements and substance abuse policy
- For current drivers:
- Annual declaration of violations (to be completed by driver)
- Obtain a copy of driving record from state agency (every year)
- Send drivers for randomly selected drug and alcohol screens
- For terminated drivers:
- Document files with termination paperwork
- Remove them from testing pool
- Maintain a calendar of expiration dates for current driver medical cards and licenses
- Maintain records of random drug and alcohol testing, as well as results
- Communicate with Mary regarding drivers to be added or removed from DMP insurance (secure appropriate info from drivers if needed)
- Notify of any accidents that may need to be reported
- Maintain records of driver training (dates and attendance)
- Conduct mock audits (when needed)
