Overview

Join our compassionate healthcare team as a Licensed Practical Nurse (LPN), Certified Nursing Assistant (CNA), Gerontology Nursing Assistant (GNA), or Companion caregiver, dedicated to providing exceptional patient-centered care. Duties

  • Monitor and document patient observations, including vital signs, medication administration, and responses to treatments using Electronic Health Record (EHR) systems.
  • Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary.
  • Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care techniques and managing behavioral challenges.
  • Support patients with medical procedures such as catheter care, wound dressing changes, and hospice or palliative needs while adhering to HIPAA regulations.
  • Prepare nutritious meals and assist with meal planning and preparation to meet dietary needs of residents or clients.
  • Maintain a clean environment by performing light housekeeping tasks like laundry and general cleaning to promote a safe living space.
  • Accurately complete medical documentation and records, ensuring compliance with healthcare standards and privacy laws.
  • Collaborate effectively with interdisciplinary teams using EMR/EHR systems to ensure seamless patient care coordination.
  • Assist with transfers and mobility using equipment such as Hoyer lifts or other assistive devices while ensuring safety protocols are followed.
  • Engage in patient observation to detect changes in condition early and communicate findings promptly to supervising healthcare professionals.

Skills

  • Strong knowledge of medical terminology, medication administration protocols, and patient care procedures.
  • Experience working in assisted living facilities, nursing homes, or home health environments with a focus on senior care or individuals with disabilities.
  • Proficiency in EMR/EHR systems for documentation and communication of patient information.
  • Ability to perform ADL assistance with compassion and professionalism while respecting patient dignity.
  • Excellent observational skills for monitoring vital signs, behavioral changes, or symptoms related to dementia or hospice needs.
  • CPR certification and familiarity with emergency response procedures to ensure safety at all times.
  • Experience working with individuals who have developmental disabilities or require specialized memory or dementia care.
  • Capable of handling medical equipment such as catheters, Hoyer lifts, or other mobility aids safely and effectively.
  • Knowledge of HIPAA regulations to maintain confidentiality of all patient records and information.
  • Flexibility to work live-in shifts or part-time schedules within home & community settings as needed.
