About Dash Connected Care
Dash Connected Care is a growing Remote Patient Monitoring company partnering with clinics to help manage chronic conditions and improve patient outcomes through connected health technology.
Key Responsibilities
- Monitor patient data and alerts through our RPM platform.
- Review alerts generated from connected devices (such as blood glucose, blood pressure, or weight) and respond according to clinical protocols.
- Communicate with clinic staff to address patient issues, escalations, or care needs identified through monitoring.
- Proactively review patient data and conduct monthly patient check-ins.
- Contact patients to discuss their health data, provide basic coaching, and answer questions according to the guidance provided by the partnering clinics.
- Maintain accurate documentation of patient interactions and monitoring activities within the RPM system.
Training and Support
- Comprehensive training will be provided on all workflows and the monitoring platform.
- New hires will work alongside an experienced MA during their first few weeks to ensure a smooth onboarding process.
Pay: $15.00 - $19.00 per hour
