Position: Case Manager
Position Type: Part-Time / As Needed (1099 – Non-W2)

Position Overview
We are seeking compassionate, motivated individuals who feel called to support others in overcoming barriers in receiving public assistance to include but not limited to: SNAP, Medicaid and/or Medicare, Housing, and legal barriers.
No prior case management experience needed. If not certified, we will provide the courses and exam needed to become certified within 30 days of hire.
Training & Compensation Structure
Probationary Phase/Getting Certified
- Base Pay: $18.00 (while on the clock)
- During this period, you will not work directly 1:1 with clients but will shadow case management duties upon clients consent for a minimum of 6 sessions. .
- Completing company required training (paid by company)
- Community outreach (with another coach, or management)
- Perform client intakes upon completion of training our intake process.
- Shadowing and professional development
This phase prepares you for unsupervised case management work. Probation period lasts a minimum of 30 days to ensure time to for all your training.
Post-Probationary Phase/ Actively Certified
- Base Pay: $22.00 DOE (while on the clock)
- Provide 1:1 case management support to your assigned case load
- Provide outreach supply deliveries with a coach or another employee
- Complete intakes
- Maintain and enter case notes within 24 hours of meeting with your client
- Provide follow-up on coach sent referrals for services outside our scope of practice
Requirements
- No prior case management experience required
- Background check required (paid by company) per federal regulations of working with vulnerable populations.
- No drug testing required
- Must adhere to a strict no-tolerance policy for substance use while working
- Remain HIPPA compliant at all times and only contact clients during business hours

*Due to providing clients for you, and client leads with a base pay, we ask that for the first 6 months of contracting with us to help offset costs that a total of $100 a month is paid by the first of each month to help with clinic upkeep costs. Included with the $100 is your own office space inside our clinic suite, lobby access, private key to employee only elevator, and 24/7 access to all the amenities in our clinic to include any supplies needed while meeting with your case load.
