RESPONSIBILITIES
Under the professional supervision of the Prevention Program Manager, the Infant and Child Case Manager provides assessment, linkage, and advocacy services to individuals from birth to seven who are at risk of serious emotional disturbance. The Infant and Child Case Manager will act as the primary facilitator in coordinating and integrating services for the assigned caseload; develop and implement Individual Service Plans (ISPs); complete required documentation; and comply with all applicable regulations governing program services. Applies generally prescribed standard practices over a wide range of procedures with some latitude for decision making.
REQUIREMENTS
Minimum Requirements
M1: Bachelor’s Degree in Psychology, Social Work, Criminal Justice, Vocational Rehabilitation, Counseling, Sociology, or other approved Human Services degree
OR
Registered Nurse with a valid Registered Nurse license to practice in Virginia
M2: Valid driver’s license, safe driving record, and reliable transportation
Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.
ANNUAL SALARY RANGE
$43,722 - $76,51
