What are the responsibilities and job description for the Home Health Marketer position at Danny's Home Health Care Inc.?
The Home Health Marketer identifies, develops, and maintains relationships between the agency and hospitals, nursing homes, physicians, and community organizations that result in the acquisition of patient referrals for the agency.
Essential Expectations and Procedural Responsibilities:
-Coordinate services for patients discharged from hospitals and other referral sources
-Obtain valid Medicare and other referrals as outlined in offer letter
-Identify patients PCP and obtain authorization for home health services
-Obtain doctor signatures for various home care orders and document accordingly
-Assist in development of patient care plan and coordinate home care needs
-Other duties as assigned by supervisor
-Must be able to pass a background check
Job Type: Full-time
Pay: $50,000.00 - $90,000.00 per year
