Role Description This is a full-time, regional on-site Nurse Practitioner role based in Jamaica, and Ronkonkoma, NY. The Nurse Practitioner will provide comprehensive primary care services, including assessing patients, taking medical histories, performing physical examinations, ordering and interpreting diagnostic tests, and developing evidence-based treatment plans. Daily responsibilities include managing acute and chronic conditions, prescribing medications, delivering health education, and coordinating care with physicians and other members of the interdisciplinary team. The role involves documenting all encounters in the electronic health record, ensuring adherence to clinical protocols, and supporting quality improvement initiatives. The Nurse Practitioner will also help address social determinants of health, advocate for underserved patients, and contribute to a culturally responsive care environment.
Qualifications
- Current Nurse Practitioner license and certification in the State of New York, with appropriate national board certification (e.g., FNP, ANP, or equivalent).
- Demonstrated clinical skills in primary care, including assessment, diagnosis, treatment planning, and management of acute and chronic conditions.
- Experience working with diverse, underserved populations and commitment to culturally sensitive, language-inclusive care.
- Strong communication, interpersonal, and teamwork skills to collaborate effectively with physicians, nurses, behavioral health, dental, and support staff.
- Proficiency with electronic health records and documentation practices, along with solid organizational and time-management skills.
- Ability to function autonomously within scope of practice, exercise sound clinical judgment, and follow FQHC and regulatory guidelines.
- Master’s degree in Nursing or related field required; prior FQHC or community health experience is highly preferred.
- Willingness to participate in quality improvement, patient education initiatives, and ongoing professional development.
