Job Description
Under general supervision, performs advanced public health investigation and follow-up activities to prevent the spread of infectious diseases. Conducts case and contact investigations, field visits, and clinical support activities requiring specialized training and judgment. Administers and interprets tests, delivers results, schedules clinic appointments, interviews clients, and provides education and counseling for infection and disease. Works closely with clinical staff to ensure timely diagnosis, treatment initiation, and continuity of care, and collaborates with community partners to support disease prevention and control efforts.
Responsibilities
Conducts case and contact investigations, including interviewing patients and contacts to identify potential exposures, risk factors, and barriers to care. Ensures accurate documentation of findings in medical and public health records. Administers, reads, and documents diagnostic tests in clinic and field settings; delivers results in accordance with program protocols; schedules follow-up evaluations and medical appointments as needed. Provides patient education and counseling regarding infection, disease, prevention, treatment adherence, and public health requirements. Supports screening activities in shelters, clinics, and other high-risk settings, coordinating with partner organizations to facilitate testing, evaluation, and treatment initiation. Conducts field visits and follow-up activities to ensure continuity of care, treatment completion, and timely evaluation of contacts; may perform venipuncture and specimen transport in support of disease testing. Prepares reports and data summaries for program management, including case status updates, contact follow-up outcomes, and service delivery metrics. Performs other duties as assigned.
Qualifications
Education, Experience and Training: Graduation from an accredited high school/GED program. Four (4) years job-related experience or 60 hours of college or a combination of the two. Experience in case management, performing venipuncture, and specimen transport preferred. Special Requirements/Knowledge, Skills & Abilities: Skilled in the use of standard software applications. Must possess a valid Texas Driver’s License, with a good driving record. Ability to drive to various site locations. Physical/Environmental Requirements: Work requires normal sitting and standing and generally the occasional lifting of 25 pounds or less.
