JOB DESCRIPTION:
Responsible for assisting patients with economic, environmental, social, and psycho-social adjustment difficulties in order to obtain maximum medical care. Assesses patients' medical social, and psychological situation, determines appropriate intervention methods and counsel patients and their families to assist them in adjusting to changes in their social roles and environment due to medical reasons. Facilitates discharge planning with outside agencies and assists with financial arrangements. Reviews patients' charts and confers with appropriate medical and nursing staff to gather pertinent data necessary for evaluation of patients' needs.
EDUCATION:
BSW
EXPERIENCE:
Two years experience in mental health preferred.
