Responsibilities:

Order prescribed medications, supplies, and equipment as needed to support resident care

Follow individualized care plans tailored to each resident’s unique needs

Perform assigned duties including wound care, treatments, and clinical procedures with precision

Accurately document all observations, interventions, and resident evaluations

Maintain strict confidentiality and uphold residents’, families’, and facility’s rights in accordance with HIPAA regulations
Requirements:

Valid LPN license in the State of New York

Experience in Skilled Nursing preferred but not required
