Join a growing medical practice in beautiful SWFL. Cypress at Home is seeking a Primary Care Physician (MD/DO) to provide highly personalized care at both Cypress Cove, our affiliated Life Plan Community within the HealthPark Florida campus, and our concierge practice where our clients receive extended appointment times and exceptional access.

WHAT YOU WILL BE DOING:

• Deliver comprehensive concierge primary care with emphasis on preventative medicine, chronic disease management, wellness planning, and proactive client engagement.
• Maintain a smaller panel to support extended appointment times and enhanced access through in-person, phone, and digital communication.
• Support future program growth including possible multi-physician model development as Cypress at Home expands services to additional sites and a collaborative physician team.
• Participate in the development of future concierge practice locations and service line expansion.
• Contribute to practice standards, client experience, and quality initiatives.
• Coordinate with specialists and hospitals to ensure seamless care transitions.
• Participate in quality initiatives, protocol development, and operational planning.
• Support client enrollment, retention strategies, and community outreach.
• Serve as a clinical resource to nursing and administrative staff.
  
  
  

WHAT WE ARE LOOKING FOR:

• Board-certified Internal Medicine or Family Medicine physician.
• Experience with, or strong interest in, concierge medicine, personalized care models, or private-practice style settings.
• Geriatric experience preferred.
• Compassionate, relationship-driven, and comfortable providing a high-touch patient experience.
• Comfortable practicing in both a solo-leadership capacity or as part of a multi-physician team.
