ESSENTIAL JOB FUNCTIONS:
Direct patient care provided in an integrated behavioral health care model. Services to be provided to a diverse and underserved clinical patient population of all ages.
Completes comprehensive assessment within program guidelines and formulates a diagnostic impression by conducting client and /family interviews and identifying strengths, weakness, problems, and needs, for the development of a treatment plan.
Provide evidence-based treatment for individuals, families, and groups, to include crisis intervention and case management.
Appropriate, adequate, and timely documentation within the electronic medical record, complete electronic documentation of all clinical contacts within 72 hours
Participate in CVHS organizational, clinical, and quality improvement meetings representing one’s practice site and behavioral health.
Participate in all meetings relevant to the clinical practice site (e.g., provider meetings, staff meetings, quality assurance committee, health center week planning)
Develop and maintain working relationships with local organizations that can collaborate to improve the continuum of care for patients (e.g., attend local service provider networking meetings to learn about local resources and share information about CVHS services with other organizations)
Supports a team-based approach to patient-centered medical care.
KNOWLEDGE, SKILLS, AND ABILITIES:
Proven psychiatric medication management capabilities
Willingness to continuously learn and update clinical skills
Qualifications:
EDUCATION, TRAINING AND EXPERIENCE:
Meets professional and legal standards for clinical practice in a primary care setting.
Licensed to practice as Psychiatric Nurse Practitioner in the state of Virginia
Eligible for reimbursement from third party payers
Three years prior practice experience in a public behavioral health or medical setting
Familiarity with integrated behavioral health care, brief and problem focused therapies
Mental Health first aid certification (can be obtained after employment)
