Job Responsibilities:
- Provide compassionate care to our patients, offering support, comfort, and symptom management across both hospice and palliative care settings.
- Collaborate with physicians and the interdisciplinary team to develop personalized care plans tailored to each patient’s needs.
- Educate patients and families on disease progression, symptom management strategies, and the overall care process.
- Coordinate and administer medical treatments to ensure patient comfort and symptom control.
- Approach all duties with a heart for hospice and palliative care, delivering services with empathy, dignity, and respect.
Qualifications:
- Must be a fully licensed Nurse Practitioner
- Minimum 1 year Hospice experience
- Experience in palliative care is a plus but not required.
