JOB SUMMARY:

This role is a trusted member of the Recuperative Care team, delivering health and support services for Curry Senior Center’s Recuperative Care Program.  This role increases our patient’s health, health knowledge and self-sufficiency through a range of activities such as coordinating health care appointments, coordinating food distribution for patients and tenants of the building in which our Program is housed, providing outreach, and social support.  This role assists older adults in ensuring they receive health care, health education.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

·        Actively participate as a critical member of the Recuperative Care team to increase patient well-being, program success, and team cohesion.

·        Contact patients and provide reminders, ensuring they attend their scheduled medical appointments. 

·        Contact patients if they miss meals to assess needs and help overcome barriers. 

·        Outreach to patients whom the Team has identified as needing social support. 

·        Support in the provision of health education to individuals or groups on topics related to health and health promotion, food insecurity, hypertension, diabetes prevention, diabetes testing, and supportive services.

·        Support the coordination of the distribution of food. 

·        Support and lead patient social events.

·        Provide information and support Case Manager(s) in providing resources necessary to improve health or address health-related social needs. Including, but not limited to, food and nutrition, housing security, economic support, and legal services. 

·        Collect data and information as required by the treatment plan and Program processes. 

·        Participate in Program team meetings and provide feedback to improve the Program.

·        Use assigned technology to complete job functions effectively. 

·        Maintain and document confidential information.    

·        Performs other duties as assigned.

 

JOB REQUIREMENTS:

·        Associates Degree or equivalent experience.

·        Community Health Worker (CHW) Certification or willingness to become certified within 1 year of hire.  

·        Flexibility in working hours, with the ability to work at least one weekend per month.

·        Working knowledge of Electronic Health Record and/or client tracking systems.

·        Trained and capable to teach basic health, sanitation, and hygiene.

·        Working knowledge of basic concepts of health promotion and disease prevention.

·        Experience in community work, education, health care, or related fields preferred.  

·        Demonstrated self-awareness in terms of understanding one’s own race, culture, identities, biases, power, privileges, and stereotypes.

·        An understanding of the diverse academic, socioeconomic, cultural, disability, gender identity, sexual orientation, and ethnic backgrounds present in the community.

·        Sensitivity to populations most highly affected by homelessness and other at-risk populations. 

·        Effective verbal and written communications skills with the ability to express ideas in a clear, thorough, concise manner.

·        Strong interpersonal, communication and organizational skills. 

·        Proof of negative or nonactive TB required. Curry Senior Center will provide or reimburse for the cost of a TB test after an offer of employment has been accepted by both parties. Proof of a negative TB test within 90 days will also be accepted.

·        Effective oral and written communications skills with the ability to express ideas in a clear, thorough, concise manner.

·        Strong interpersonal, communication and organizational skills.  Solid aptitude for customer service.

·        High degree of skill in grammar, spelling and punctuation.

·        Ability to maintain the confidentiality of all information obtained.

·        Strong organizational skills and the ability to prioritize workflow.

·        High level of analytical problem-solving skills.

·        Excellent time management skills, including the ability to manage self-development.

·        Must be action-oriented and have the ability to work independently and as a team player.

·        Strong computer skills such as Chrome, Google suite, Apple iOS, Office suite.

 

 

 

PHYSICAL REQUIREMENTS/DEMANDS:

The incumbent is in a non-confined office-type setting in which they are free to move about at will. The Environment is mostly clean and comfortable but may include some minor annoyances such as noise, odors, drafts, etc. Work will be performed utilizing a computer, sitting at a desk/chair or standing while speaking and interacting with clients. Position will require average finger dexterity to operate normal office equipment and systems.

Bending:         2.5 % of the workday

Twisting:          2.5 % of the workday

Lifting:             5.0 % of the workday/Moderate, generally not over 10 pounds

Carrying:        15 % of the workday/Moderate, generally not over 10 pounds

Climbing:       10 % of the workday

 

The incumbent must be able to perform this job safely, without endangering the health or safety of themselves or others.
