Key Responsibilities
In this role, you will be responsible for administering direct patient care services within the Post-Acute Care setting. This includes conducting clinical interventional visits, evaluating residents' changes in condition, reviewing medications and orders, and coordinating care with collaborating physicians. You will also conduct Annual Wellness Visits on long-term Nursing Facility residents and may perform Annual History and Physicals as requested by Facilities.
