Summary:

We are seeking a skilled and dependable Medical Assistant – Scribe to support our rounding physician in skilled nursing and senior living facilities. In this unique hybrid role, you’ll provide both clinical and administrative support—serving as the physician’s right hand during daily rounds to ensure seamless documentation, care coordination, and patient flow.

You’ll be a vital member of our interdisciplinary care team, helping improve the health and quality of life of older adults through proactive, team-based care.

Essential Duties & Responsibilities:

Clinical Support

• Accompany the physician on rounds within assigned skilled nursing and senior living facilities.
• Prepare for each day’s patient list by reviewing charts, recent labs, and treatment plans.
• Take and record patient vitals, update medication lists, and document relevant health information.
• Assist with care coordination, follow-up orders, and communication between facilities, families, and the clinical team.
• Maintain accurate and up-to-date patient records and assist with transitions of care as needed.
  
  

Scribing & Documentation

• Accurately document the physician’s findings, assessments, and treatment plans in real time within the electronic medical record (EMR).
• Record all procedures, diagnostic tests, and consult notes as directed by the provider.
• Ensure documentation meets all compliance, coding, and quality standards.
• Support completion of clinical paperwork, orders, and follow-up notes after rounding sessions.
  
  

Administrative Support

• Coordinate with facility nursing staff and office teams to schedule follow-up visits, referrals, and tests.
• Communicate effectively with patients, caregivers, and facility partners to ensure continuity of care.
• Support quality initiatives, documentation reviews, and process improvements as assigned.
  
  

Qualifications:

• High school diploma or equivalent required; completion of an accredited Medical Assistant program strongly preferred.
• Certified or Registered Medical Assistant (CMA, RMA, or CCMA) preferred.
• 1 year of experience in a clinical or long-term care setting; experience supporting rounding providers preferred.
• Proficiency with EMR systems and medical scribing.
• Strong attention to detail, excellent communication skills, and ability to multitask in a fast-paced care environment.
• Compassionate and professional approach to working with older adults and interdisciplinary care teams.
• Reliable transportation required for travel between assigned facilities.
