What are the responsibilities and job description for the Early Intervention Case Manager - Part-time position at Cumberland Mountain Community Services Board?
Cumberland Mountain CSB is seeking a professional for an exciting career as a Part-time Early Intervention (EI) Case Manager. The EI Case Manager will provide service coordination to developmentally delayed infants and toddlers who reside in Buchanan, Russell and Tazewell Counties. Duties include: Screening for services, oordinate and monitor the delivery of IFSP (Individualized Family Service Plan) Supports and Services, make direct monthly contacts with families by telephone or face to face, request needed information from physicians and other service providers, document all contacts made and all activities completed with or on behalf of children and families, hold IFSP reviews as needed, and monitor Medicaid and insurance coverage issues. Rate of pay is based on education and experience. Will work up to 29 hours per week, depending upon need.
Hiring range: $17.50- $26.25 hourly. (Offers for candidates with more than 20 years of relevant experience may exceed the listed hiring range. Cumberland will only engage in compensation discussions and negotiations after an initial offer of employment is extended.)
