Cumberland Mountain Community Services Board was formed in 1972 as a Chapter 10 Community Services Board under Virginia law. The organization provides mental health, intellectual disabilities, and substance abuse treatment services to residents of Buchanan, Russell, and Tazewell Counties.
This position serves as a liaison and discharge planner for the Southwest Virginia Mental Health Institute (SWVMHI) and all state psychiatric facilities. Responsibilities include participating in treatment team meetings, developing and implementing discharge plans, maintaining a caseload of individuals in state psychiatric facilities, coordinating services and follow-up care with outpatient programs, and attending regularly scheduled staff meetings.
Key Responsibilities:
- Develop and implement discharge plans for individuals transitioning from state psychiatric facilities.
- Maintain a caseload of individuals receiving services from state psychiatric facilities.
- Coordinate services and follow-up care with outpatient programs.
- Participate in treatment team meetings at SWVMHI and other state psychiatric facilities.
- Attend regularly scheduled staff meetings to coordinate discharge planning and community reintegration efforts.
Requirements:
- Bachelor's Degree in Human Services field.
- Effective problem-solving and decision-making skills.
- Ability to intervene in crisis situations.
- Good written and oral communication skills.
- Knowledge of local human service resources.
- Ability to use independent judgment and discretion in assigning duties.
- Good organizational and time management skills.
- Good computer skills and ability to effectively use electronic health record systems.
- Current valid driver's license and good driving record.
Working Conditions:
- Frequent travel to meetings and other locations is required.
