To provide comprehensive primary care services to patients of all ages, focusing on health promotion, disease prevention, and management of acute and chronic conditions. The APRN will work collaboratively within a healthcare team to deliver high-quality, patient-centered care.
Job Functions/Responsibilities:
- Obtain comprehensive medical histories and conduct physical examinations.
- Diagnose and treat common illnesses and injuries.
- Order and interpret diagnostic tests (e.g., lab work, imaging studies).
- Prescribe medications and other treatments as needed.
- Develop and implement individualized care plans.
- Counsel patients on preventive health measures, such as vaccinations and healthy lifestyle choices.
- Provide education to patients and their families regarding their medical conditions and treatment plans.
- Make referrals to specialists and other healthcare providers as appropriate.
- Collaborate with other members of the healthcare team, including physicians, nurses, and other support staff.
- Maintain accurate and up-to-date patient records.
- Stay current with the latest medical advancements and best practices.
- Participate in quality improvement initiatives.
- Adhere to all applicable regulations and guidelines.
- Demonstrate a commitment to providing compassionate, patient-centered care.
- Other duties as assigned.
Qualifications
Required Experience:
- Experience working with diverse patient populations.
- Experience with electronic health records (EHR) systems.
Required Skills and Abilities:
- Comprehensive knowledge of family medicine principles, practices, and procedures.
- Ability to assess, diagnose, and treat patients across lifespan.
- Excellent communication, interpersonal, and patient education skills.
- Strong clinical decision-making and problem-solving skills.
- Ability to work independently and as part of a team.
- Proficiency in performing physical examinations and ordering appropriate diagnostic tests.
- Ability to manage chronic conditions and provide preventive care.
- Knowledge of current healthcare regulations and guidelines.
- Cultural sensitivity and awareness.
- Commitment to providing high-quality, patient-centered care.
Required Education:
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Current and unrestricted Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) licenses in the state of Kentucky (or eligibility for licensure)
Schedule Requirements:
PRN position, weekends ONLY, 100% clinic-based, in person work required.
Work schedules are set at the discretion of CFMC and may be adjusted at any time based on the operational needs of the organization.
