Job Summary
Cultiva La Salud is looking for dedicated Community Health Workers who can demonstrate cultural and linguistic competency. The Community Health Worker (CHW) plays a vital role in bridging the gap between care providers and the community. This position involves working closely with individuals and families to provide education, support, and resources aimed at improving health outcomes. The CHW will engage with diverse populations, focusing on promoting public health initiatives and addressing social determinants of health.
Duties
- Conduct outreach to educate Fresno County community members about available health services and resources.
- Assist individuals in navigating healthcare systems, including Medicare and other public health programs.
- Provide support for counseling and recovery services, facilitating access to appropriate resources.
- Utilize motivational interviewing techniques to empower clients in making informed health decisions.
- Offer detailed case management to individuals and families, accurately assessing their needs.
- Coordinate care and services, helping community members identify and access resources while overcoming barriers.
- Complete appropriate referrals through household-based risk assessments.
- Facilitate the development of individual and/or group action plans and assist in goal attainment.
- Provide ongoing case management for families, following up to ensure they have connected with necessary resources.
- Document and track participant cases using a database, collecting and analyzing data related to community health needs and program effectiveness.
- Collaborate with healthcare professionals, including nurses and social workers, to develop comprehensive care plans and endure coordinated care.
- Organize and participate in public events, including door-to-door outreach, tabling, and community activities to raise awareness about health issues affecting the community.
- Develop educational materials tailored to the needs of specific populations regarding nutrition, wellness, and preventive care.
- Preform other duties as assigned.
Requirements
- High School diploma or General Education Degree (GED); or one to three months of related experience and/or training; or equivalent.
- Experience in case management for families is a plus.
- Background in public health, social work, or a related field preferred.
- Bilingual (reading, writing, and speaking) in English and Spanish is preferred.
- Knowledge of Medicare services and other relevant healthcare programs is beneficial.
- Strong public speaking skills with the ability to engage diverse audiences effectively.
- Proficiency in motivational interviewing techniques to facilitate client engagement.
- Excellent communication skills, both verbal and written, with an emphasis on educating others.
- Ability to collect and accurately analyze data for reporting purposes.
- Information technology skills, including proficiency with laptops, smartphones, Google Workspace, web-based applications, Microsoft Office (Word and Excel), etc.
- Reliable transportation, a valid driver’s license, and insurance required for the vehicle used for work purposes (employee must be listed on the insurance).
- A passion for community service and improving health outcomes for underserved populations.

Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
