CTF Illinois is seeking a dedicated and organized QIDP / Case Manager to support individuals with developmental disabilities in our Community Day Services Program at our Tinley Park location. Position Overview
- Schedule: Monday–Friday (daytime hours) with a flexible, hybrid schedule (in-person and remote)
- Develop and implement individualized strategies based on each person’s goals, preferences, and personal plan
- Coordinate services by maintaining regular contact with individuals on your caseload
- Participate in discovery and interdisciplinary team meetings
- Complete required assessments, documentation, and consents
- Monitor progress and outcomes through monthly reviews and reporting

Qualifications
- Bachelor’s degree in a human services-related field (required)
- Must meet certification and training requirements for QIDP
- At least 1 year of experience working with individuals with developmental disabilities
- Valid driver’s license with an acceptable driving record
- Strong organizational skills and ability to work independently
- QMRP/QIDP certification preferred
- Previous DSP training a plus
