Job Duties
- Work hours should coincide with Day Program operation hours, which
be subject to change, per supervisor request.
- Check monthly for appointments (i.e. medical, vision, dental,
- Schedule and confirm appointments, coordinate transportation needs,
- Follow-up as necessary and appropriate.
- Assist nursing to check in monthly medications if requested or
- Ensure that a medical synopsis form (completed by the nurse) is
- Return all appropriate documentation to nursing for follow-up.
- Ensure physician orders are signed by physician monthly
- Complete bi-annual medication review form and ensure physician
- Adhere to terms and conditions of employment as outlined by Employee
Handbook and job description.
- Pay Rate: \$20-21/hr based on education and experience
