Women's Health Services Prenatal Case Manager

Position Summary:

This is a direct service position in the Women’s Health Services Department that provides services to Crusader prenatal patients. The prenatal case manager will be responsible for the provision of case management services, in the clinic, including assessment and treatment planning, care coordination and health promotion, transitional care, patient and family education and support, and referral linkage to social, community, and other clinically appropriate services in coordination with the provider care team. The prenatal case manager is expected to facilitate and submit patient applications for Medicaid, WIC, TANF, and other state and federal supportive services. Appropriate community linkages and referrals are made to help the client achieve pre-determined goals. This position also interfaces with community organizations to collaborate, coordinate patient care, and promote organizational services. This position reports to the Medical Unit Manager of Women’s Health Services.

Education:

• Minimum of an associate’s degree in a human service field or equivalent experience. Relevant specialty certification and/or training preferred.
• Current with any required continuing education requirements.

Experience:

• Minimum one year of experience in the human service field, preferred.

Technical Knowledge:

• Equipment: common office machines.
• Software: previous electronic medical record experience, MS Office (Word, Excel, Outlook, PowerPoint), and internet.
