Essential Job Functions
- Develop, review, and update individualized resident care plans.
- Coordinate and participate in interdisciplinary care plan meetings with residents, families, and healthcare team members.
- Ensure resident care plans accurately reflect individual needs, goals, and changes in condition.
- Collaborate with the MDS Coordinator and interdisciplinary team to ensure accurate assessments and documentation.
- Supervise and support nursing staff and CNAs to ensure quality resident care.
- Monitor nursing practices for compliance with facility policies and state and federal regulations.
- Communicate effectively with residents, families, physicians, and facility leadership.
- Assist with quality assurance initiatives and continuous improvement of resident care.
- Support the Director of Nursing with clinical and supervisory responsibilities as needed.
- Active Alabama RN license required.
- Previous long-term care or skilled nursing experience preferred.
- Care planning and MDS experience preferred.
- Previous supervisory experience preferred.
- Strong leadership, communication, and organizational skills.
- Ability to work independently and collaboratively with an interdisciplinary team.
- Must pass background check and drug screening.
